Here’s an example
I was asked to help a mushroom substrate factory tackle the quality issues that were limiting their yield of sellable product. The quality issues were leading to dissatisfied customers and harming the reputation of the company in the market.
The team had been working for a number of months to find the causes using an iterative approach built on prior experience, supported by trial and error. Their progress was hampered by the many interdependent factors that affected the production process, which they weren’t able to separate.
I helped the team to create a structured root cause analysis of the factors affecting production, and focused their efforts on those that affected product quality. Together we grouped the factors into related clusters, and made sure to test one factor at a time. We also stopped testing factors that were unrelated to the quality issue.
With this approach, we were able to isolate some of the factors and create improvement actions to mitigate them. This enabled us to improve the yield of the product.
